What to Do as a New Patient

Fill out new patient information online or call the office at 208-291-5553 to request a link to register for the patient portal.

Once you have received the patient registration link, register for the patient portal. 

• Please be aware that the patient portal link is good for 24 hours. If it requires more time, you will need to request your portal link to be resent.

• If you are a parent signing up for your child(ren), you may use the same email address for multiple patients, but each will need a new username for the login.

Once registered:

• Sign all consents

• Fill out an intake form

• Upload a copy of insurance information to your patient portal documents.  (This is needed for diagnostic and lab orders.  These will need to be filled out prior to your first appointment.)

You will receive an email and text message with appointment details.
